What is the endothelium and what is endothelial function?
The endothelium is the single-layer lining on the inside of all blood vessels. It regulates how wide vessels open, how blood flows and how immune cells cross the vessel wall. “Endothelial function” describes how well this regulation works.
The endothelium explained simply
The endothelium is a wafer-thin layer of cells lining every blood vessel from the inside – from the main artery to the finest capillaries. Taken together it covers a substantial surface and is an active organ, not a passive wallpaper.
Its tasks include:
- Controlling vessel width: Endothelial cells make nitric oxide (NO), which relaxes smooth vessel muscle. Vessels widen and blood pressure is co-regulated.
- Keeping clotting in balance: An intact layer prevents blood from clumping unnecessarily.
- Controlling exchange: The endothelium helps decide which substances and immune cells pass from blood into tissue.
What is endothelial function?
When endothelial function is impaired, less NO is made and vessels respond more sluggishly. Contributing factors include oxidative stress and persistent inflammatory processes. In research, function is usually measured by flow-mediated dilation (FMD): ultrasound measures how much an arm artery widens after brief occlusion.
Best supported are exercise, not smoking, a balanced diet and a healthy body weight. Supplements do not replace these.
Endothelium and curcumin in studies
Studies in people have examined whether curcumin affects endothelial function. In a randomised, placebo-controlled study of 39 healthy adults aged 45 to 74, flow-mediated dilation (FMD) rose by about 36% after 12 weeks of a curcumin formulation at 2,000 mg per day, but did not change with placebo. It is discussed that curcumin might act through antioxidant and inflammation-related pathways such as the NF-κB pathway.
The context matters: these are individual, comparatively small studies with measures such as FMD, not evidence that curcumin prevents or treats cardiovascular disease. There is no authorised health claim for it. If you have cardiovascular problems or take blood-pressure medication, discuss supplements with your doctor. This text is not medical advice.
Why does research care about the endothelium?
Since the endothelium takes part in vessel width, clotting and immune responses, it is seen as the interface between blood and tissue. Changes are regarded as an early signal, long before symptoms appear. Endothelial function is therefore a popular target in studies on lifestyle, diet and plant compounds. An improved measure is however an intermediate result; whether a long-term health benefit follows must be shown separately.
Frequently asked questions
What is endothelial dysfunction? The term describes a reduced ability of the vessel lining to widen vessels and keep balance. It is regarded as an early sign of vascular change and is often used as an outcome measure in research.
Does the endothelium relate to blood pressure? Yes, through nitric oxide production it contributes to regulating vessel width. Blood pressure depends on many other factors too.
Can I measure endothelial function myself? Not reliably at home. FMD measurement is done in specialised centres and studies.
